Job summary

Colorado PACE is seeking a Home Care Registered Nurse to join our interdisciplinary team in Aurora, CO. The position is full-time, schedule Monday–Friday, 8:00 a.m. to 4:30 p.m., with an annual pay range of $81,244 to $95,451.

The RN will coordinate care in patients' homes, provide education, and ensure continuity of care across services. We support a strong culture with comprehensive benefits including mileage reimbursement, relocation assistance, employer-funded health plan and HSA, paid time

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• One year of professional experience required, minimum one year experience with frail/elderly population, home care or geriatric experience.
• Valid RN License. Valid Colorado Driver’s License and proof of Auto Insurance in accordance with state statute.

Responsibilities

• Provides patient education and training.
• Collaborates with interdisciplinary team to meet patients’ needs and maintain continuity of care.
• Participates in staff meetings, interdisciplinary team meetings, and educational opportunities.
• Collaborates with physicians and acts as liaison for care coordination of the designated caseload.
• Assesses patient needs and provides services within scope of practice and individualized plan of care.
• Documents in the electronic record within stated standards; provides complete and accurate documentation.
• Participates in infection prevention and control and environmental safety procedures.

Annual Pay Range: $81,244 - $95,451

Supplemental Pay: Based on position, schedule and/or availability: Paid Mileage/Shift Diff/Stipends/Overtime

MILEAGE AND EXPENSE REIMBURSEMENT:Sixty-two and a half cents per mile – one of the highest in the industry! Relocation Reimbursement.

REWARDING WORK YOU WILL DO:

This Registered Nurse position has the primary responsibility to participate as a member of the interdisciplinary team to assess, plan, implement and evaluate care provided in the patient/participants place of residence. This nurse will actively participate in coordination of all aspects of patient’s care, in accordance with current professional standards and practice. This position will also require a flexibility to meet patient care needs within the continuum of services.

ESSENTIAL DUTIES:

• Provides patient/caregiver education and training specific to identified needs.
• Collaborates in the provision of care, treatment and services with the interdisciplinary team to meet patients’ needs and maintain continuity of care between disciplines, programs, services and with other organizations as needed.
• Participates in agency activities to include interdisciplinary team meetings, staff meetings, and other committee meetings as assigned and educational opportunities.
• Collaborates with the physician(s) and acts as liaison for the care coordination of the designated case load.
• Assesses patient needs and provides services within scope of practice and based on individualized plan of care appropriate to the patient’s needs, strengths, limitations and goals.
• Communicates/collaborates with contracting providers regarding services provided.
• Assesses for discharge and/or transfer needs and facilitates the process to ensure that continuity of care, treatment and service is maintained.
• Performs clinical duties within the nursing scope of practice.
• Documents in electronic record within stated standards provides; complete and accurate supporting documentation.
• Participates in the surveillance, prevention and control of infection.
• Participates in environmental safety and equipment management procedures.
• Utilizes information management to support decision making to improve patient outcomes, improve health care documentation, assure patient safety and improve performance in patient care, treatment and services.
• Documents in electronic record within stated standards provides; complete and accurate supporting documentation.
• Utilizes company resources in a prudent and appropriate manner. Is a good steward of company resources.
• Manages patient care within accepted standards for utilization of DME, medications and supplies.
