Job summary

Dignity Health - Marian Regional Medical Center in Santa Maria, CA is seeking a Case Manager RN to coordinate the care and services of patients across the continuum of illness, promote effective utilization of resources, and lead the interdisciplinary team to achieve optimal outcomes through case management.

You will perform admission assessments, screen appropriateness, develop discharge plans, evaluate post-discharge needs, and collaborate with patients and families to obtain desired outcomes.

Qualifications

• Minimum 1 year RN experience.
• CA RN license required.
• BLS-CPR within 30 days of hire.
• Driver's License and CA auto insurance required.

As our Case Manager RN, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.

Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs.

To be successful in this role you must be knowledgeable in community resources and alternate care facilities.

Job Requirements

• Minimum of One (1) year of RN experience
• Registered Nurse: CA RN License
• Basic Life Support - CPR (BLS-CPR) within 30 days
• Driver's License: CA (AUTO:CA)
• Auto Insurance (AUTO-INS)

Preferred

• Hospice or Home Health experience
