Job summary

Dignity Health - Marian Regional Medical Center in Santa Maria, CA is seeking a Case Manager RN to coordinate care across the illness continuum and promote optimal outcomes using case management tools.

Responsibilities include admission assessments, discharge planning, and resource coordination with the interdisciplinary team to ensure timely, appropriate services for patients and families.

Qualifications

• Minimum of One (1) year of RN experience.
• Registered Nurse: CA RN License.
• Basic Life Support - CPR (BLS-CPR) within 30 days.
• Driver's License: CA (AUTO:CA)
• Auto Insurance (AUTO-INS)

• Obtains all required physician orders, TARs, and authorizations for all services.
• Provides patient and family teaching necessary to meet the needs of the patient and family.

Job Summary and Responsibilities

As our Case Manager RN, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.

Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs.

To be successful in this role you must be knowledgeable in community resources and alternate care facilities.
