Job summary

Dignity Health - Marian Regional Medical Center in Santa Maria, CA is seeking a Case Manager RN to coordinate the care and services of patients across the continuum of illness, promote effective utilization of resources, and lead the interdisciplinary team to achieve optimal outcomes through case management.

You will perform admission assessments, screen appropriateness, develop discharge plans, evaluate post-discharge needs, and collaborate with patients and families to obtain desired outcomes.

Qualifications

• Minimum 1 year RN experience.
• CA RN license required.
• BLS-CPR within 30 days of hire.
• Driver's License and CA auto insurance required.

Responsibilities

• Utilizes the nursing process to provide safe therapeutic care.
• Assessment, Planning, Implementation, Evaluation, Diagnosis.
• Assists with coordination of patient-care activities/scheduling.
• Works collaboratively with patient and family to achieve desired outcomes.
• Obtains all required physician orders, TARs, and authorizations for all services.
• Provides patient and family teaching to meet needs.
