Job summary

• Provide patient and family teaching as needed.

Job Summary and Responsibilities

As our Case Manager RN, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.

Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs.

To be successful in this role you must be knowledgeable in community resources and alternate care facilities.

• Utilizes the nursing process to provide safe therapeutic care for patients
• Assessment, Planning, Implementation, Evaluation, Diagnosis
• Assists with coordination of patient-care activities/scheduling. Collaborates/cooperates to assure effective consistent management of clients.
• Works collaboratively with patient and family to obtain desired patient outcomes.
• Obtains all required physician orders, TARs, and authorizations for all services.
• Provides patient and family teaching necessary to meet the needs of the patient and family.

Job Requirements

Required

• Minimum of One (1) year of RN experience
• Registered Nurse: CA RN License
• Basic Life Support - CPR (BLS-CPR) within 30 days
• Driver's License: CA (AUTO:CA)
• Auto Insurance (AUTO-INS)

Preferred

• Hospice or Home Health experience
