Job summary

Home Recovery Home Aid is seeking a licensed Registered Nurse to conduct comprehensive in-home assessments and provide skilled nursing care under physician-approved care plans. You will accept admissions, perform treatments, and monitor client status, coordinating with leadership to ensure quality outcomes.

The role requires strong communication and organizational skills, CPR certification, and the ability to work with minimal supervision in a home-health setting.

Qualifications

• Holds a current Registered Nurse license.
• Graduate from an approved school of nursing.
• Minimum of two years of acute care experience requiring heart, lung, and physical assessments or equivalent education.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Ability to function with minimal supervision.
• Ability to manage multiple projects and changing priorities.
• Ability to employ critical thinking skills.
• Eligibility for certain benefits may depend on employment status.

Responsibilities

• Completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission and continued service to determine needs and outcomes.
• Accepts client admissions as assigned by Agency and weekend on-call assignments.
• Provides comprehensive nursing care through home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatments requiring physician-ordered skills.
• Observes signs and symptoms and reports them to the VP of Clinical Services/Director of Professional Services and/or physician.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet of clients as ordered by physician.
• Educates patients and families on medication and procedures.

Responsibilities

• Completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.
• Accepts client admissions as assigned by Agency and weekend on-call assignments.
• Provides comprehensive nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatment(s) requiring specialized skills ordered by the physician.
• Observes signs and symptoms and reports them to the VP of Clinical Services / Director of Professional Services and/or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet of clients as ordered by physician.
• Educated patients families on medication and procedures.
