Job summary

Home Recovery Home Aid is seeking a registered nurse to conduct initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission and ongoing service. Assessments may include OASIS items at appropriate time points.

The RN will admit clients, provide comprehensive nursing care, perform physician-ordered treatments, monitor status changes, and educate families on medications and procedures in a home-health setting.

Qualifications

• Current RN license is required.
• Graduate from an approved school of nursing.
• Minimum of two years in an acute setting involving heart, lung, and physical assessments or demonstrated equivalent training.
• Current CPR certification.
• Excellent communication, organizational, and human relations skills.
• Ability to function with minimal supervision.
• Ability to manage multiple projects and changing priorities.
• Ability to employ critical thinking skills.

Responsibilities

• Accepts client admissions as assigned and on-call duties on weekends.
• Provides comprehensive nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
• Performs treatments requiring specialized skills ordered by the physician.
• Observes signs and symptoms and reports them to leadership and/or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet as ordered by physician.
• Educates patients and families on medications and procedures.

The RN completes initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission to and continued service from the Agency to determine needs, effectiveness of care provided, and achievement of anticipated outcomes. Assessments may include OASIS items at appropriate time points.

• Accepts client admissions as assigned by Agency and weekend on-call assignments.
• Provides comprehensive nursing care through the utilization of the home assessment, nursing diagnoses, and physician-approved care plans.
• Performs treatment(s) requiring specialized skills ordered by the physician.
• Observes signs and symptoms and reports them to the VP of Clinical Services / Director of Professional Services and/or physician as appropriate.
• Notes changes in physical or emotional status of clients.
• Assesses nutritional needs and diet of clients as ordered by physician.
• Educated patients families on medication and procedures.

Qualifications

• Holds a current Registered Nurse license
• Is a graduate from an approved school of nursing.
