Job summary

Community Care, Inc. is seeking a compassionate Registered Nurse (RN) to provide skilled home health care to clients in the Westmoreland and Fayette County area. You will build meaningful connections and deliver personalized care in a supportive home setting.

The role requires PA RN licensure, CPR, and at least 1 year of nursing experience (home health preferred). Weekend Saturday–Sunday shifts with flexible scheduling and a dedicated, supportive team.

Qualifications

• Current Registered Nurse license in Pennsylvania.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience (Home Health experience preferred).
• CPR certification (required).
• Reliable transportation and a valid driver's license.
• A caring heart, strong independence, and a passion for pediatric care.

Responsibilities

• Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments).
• Develop and implement individualized care plans.
• Educate patients and caregivers on managing conditions and medications.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Document visits and patient progress accurately and timely.
