Job summary

Community Care, Inc. is seeking a compassionate RN to provide home health care in the Westmoreland and Fayette County area. This role involves skilled nursing visits, care plan development, and patient education in a supportive home setting.

The position offers weekend/day shifts, a $2,000 sign‑on bonus, weekly pay, paid orientation and training, PTO and flexible scheduling, a 401(k), and full-time benefits with on‑call clinical support.

Qualifications

• Current RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience (Home Health preferred).
• CPR certification is required.

Responsibilities

• Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, treatments).
• Develop and implement individualized care plans.
• Educate patients and caregivers on managing conditions and medications.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Document visits and patient progress accurately and timely.
