UPMC Home Health is seeking a full-time Registered Nurse for Crafton, Carnegie, Greentree and surrounding areas in Western Pennsylvania. Daylight shifts with on-call rotations and weekends; flexible shifts available.

The role involves establishing care plans with physicians, coordinating across a health care team, and teaching patients and families to achieve optimal outcomes. Ideal candidates hold an RN license with BLS/CPR; BSN preferred; drivers license required.

Qualifications

• Coordinate case management across a multi-disciplinary team.
• Deliver and evaluate patient- and family-centered health teaching.

Responsibilities

• Establish patients’ plans of care in collaboration with physicians, patients, and UM professionals.
• Coordinate case management across a multi-disciplinary health care team.
• Initiate, deliver, and evaluate patient- and family-centered health teaching.
• Strive to help patients achieve optimal health outcomes with compassionate care.
• Complete all required documentation per regulatory standards.
• Attend compliance training and follow organizational policies and procedures.
