Job summary

Personal Touch Home Care is seeking an RN to plan, organize and direct home care services, with emphasis on pediatric care for patients from birth through late teens and a focus on community health education. Ideal candidates will have experience with Adults/Geriatric populations and Special Needs, and pediatric experience is a plus, with a valid NY RN license.

Pay is per visit, with ranges $50 to $125 per visit, plus Oasis/Follow-up bonuses for timely notes.

Qualifications

• Bachelor's degree in Nursing required.
• Current active NY RN license required.
• Experience in home health or community care preferred.
• Pediatric experience a plus.

Responsibilities

• Performs comprehensive patient assessments and develops care plans.
• Re-evaluates patient nursing needs regularly.
• Coordinates care with physicians and family.
• Administers medications and treatments as prescribed.
• Educates patients and families on care plans and health maintenance.
• Documents care plans and updates physician as needed.
• Coordinates equipment and supplies for home visits.
