Job summary

Personal Touch Home Care is seeking a Registered Nurse for pediatric and geriatric home care in Queens, NY. The role emphasizes community health education and direct patient care across birth to late teens, with a focus on coordinated family education and care planning.

The RN will perform assessments, develop and adjust plans of care, administer medications, and collaborate with physicians. A valid NY RN license, BLS, and 1+ year of home care or acute care experience are required.

Qualifications

• Bachelor's degree in Nursing required and NY RN license.
• Pediatric/home health care experience preferred.
• Current Basic Life Support (BLS) certification required.

Responsibilities

• Completes comprehensive assessments to determine home care needs and performs physicals.
• Re-evaluates patient nursing needs and revises the plan of care as needed.
• Coordinates care with physicians, families and other health professionals.
• Administers prescribed medications and treatments.
• Counsels patients and families on nursing and related needs.
• Documents clinical notes and communications with physicians every 60 days.
• Supervises home health aides and LPN/LVN care as required.
• Ensures equipment and supplies are arranged for home visits.
