Qualifications

Bachelor's degree in Nursing from an accredited college/university.
Current active and unrestricted nursing license.
Valid driver's license.
Current Basic Life Support certification.
Minimum of 1 year of acute or home care experience.

Basic computer skills for documentation.

Responsibilities

Completes comprehensive assessments of patient and family to determine home care needs.
Regularly re-evaluates patient nursing needs.
Initiates and revises the plan of care with physician collaboration.
Administers medications and treatments as prescribed.
Counsels patient and family and provides care instructions.
Coordinates care with healthcare providers and other team members.
