Job summary

Personal Touch Home Care is seeking a Registered Nurse for pediatric and geriatric home health in the Bronx, NY. The RN will plan, organize, and direct home care services with emphasis on community health education and patient-family involvement.

Qualifications include a BSN or higher, NY RN license, pediatric/home care experience, and a valid driver’s license. Pay is per visit; benefits and growth opportunities are provided to eligible staff.

Qualifications

• Bachelor's degree of Nursing from an accredited college/university.
• Current active and unrestricted nursing license.
• Pediatric care experience in the acute and/or home care environment.
• Valid driver's license.
• CPR/BLS certification is preferred.

Responsibilities

• Performs comprehensive patient and family assessments for home care needs.
• Re-evaluates patient nursing needs and revises the plan of care as required.
• Develops and implements care plans with physician collaboration.
• Administers medications and treatments per physician orders.
• Provides patient and family health care instructions.
• Coordinates discharge planning and required follow-up.
• Supervises home health aides and LPN/LVN care where applicable.
• Maintains accurate clinical notes and physician communications.
