Job summary

Personal Touch Home Care, serving communities for over 50 years, seeks an experienced Registered Nurse to deliver pediatric and geriatric home care in the New York area. The role focuses on comprehensive assessments, care planning, and coding patient progress with physicians.

Candidates must hold a NY RN license, have a valid driver’s license, and possess BLS. Competitive per-visit pay and opportunities for professional growth are offered in a supportive home-care setting.

Qualifications

Bachelor's degree in Nursing from an accredited college/university.
Current active and unrestricted NY RN license.
Pediatric care experience in the acute and/or home care environment.
Valid driver’s license.
Current certification in Basic Life Support for Health Care Professionals.
Minimum of 1 year of recent acute or home care experience.

Responsibilities

Completes comprehensive assessments of patient and family to determine home care needs.
Regularly re-evaluates patient nursing needs.
Initiates the plan of care and revises as needed.
Develops the plan of care with the physician and family involvement.
Administers medications and treatments as prescribed by the physician.
Instructs and counsels patient and family on nursing needs.
Documents clinical notes and updates physician every sixty days.
Coordinates care with physician and other health professionals.
