Job summary

Ardent Home Health and Hospice is seeking a compassionate Registered Nurse (RN) to join our home health team in California. You will plan, organize, and coordinate home-based nursing services, with a focus on community health and patient education.

The role emphasizes collaboration with caregivers, discharge planning, and coordination of equipment and services, while supervising LVNs and HHAs to ensure high-quality patient care.

Qualifications

• Active California RN license is required.
• Minimum 1 year of direct patient care experience (home health preferred).
• Current CPR certification is required.
• Valid driver’s license with insured, reliable vehicle.

Responsibilities

• Develop and initiate individualized care plans with patients and caregivers.
• Deliver physician-ordered nursing services and implement preventive/rehabilitative procedures.
• Educate patients and caregivers to promote health and independence.
• Identify discharge planning needs and coordinate necessary services.
• Coordinate equipment and services required for patient care.
• Supervise LVNs and HHAs.
