Job summary

ABL Hospice is seeking a compassionate Registered Nurse (RN) to join our Home Health team in California. This full-time role involves evaluating patients, creating and implementing care plans, and tracking progress in the field, working closely with the Clinical Director and staff.

You will travel within the service area, document care using a company device, and collaborate to deliver patient-centered care with high standards and empathy.

Qualifications

• Current CA RN license required.
• 2 years' experience in home health or hospice care preferred.

Responsibilities

• Provide nursing care within the scope of practice per patient care plan, coordinating with the Interdisciplinary Team.
• Administer treatments, monitor patients, and report changes to physicians or case managers as needed.
• Maintain accurate clinical records and documentation using company devices.
• Supervise LVNs when assigned.
• Comply with all policies to maintain high standards of care.
