Job summary

CARE FIRST HOME HEALTH CARE INC is seeking a Registered Nurse for home health in California. The RN will assess patients at home, develop and adjust care plans, and provide ongoing nursing care and education to patients and families.

Oversight of LVNs and CHHAs ensures coordinated, quality care in the patient’s residence. The role requires CA RN licensure, CPR certification, and at least one year of relevant experience, preferably in home health or hospice.

Qualifications

• Licensed Registered Nurse in California with CPR certification.
• Minimum one year of nursing experience, preferably in home health or hospice.
• Strong interpersonal and communication abilities with patients and families.
• Proficient with computerized clinical documentation and computers.

Responsibilities

• Completes initial, comprehensive patient assessment for home care needs.
• Creates and revises plan of care with measurable goals and time frames.
• Performs ongoing reassessments as patient status changes.
• Delivers medications and treatments per physician orders within plan of care.
• Supervises LVNs and CHHAs per agency policies.
• Educates patients and families on medications and disease management.
• Coordinates care with physicians and other providers.
• Completes timely clinical notes and documents changes to the plan of care.
• Ensures patient has medications and DME needed for safe care.
• Maintains HIPAA compliance and participates in quality initiatives.
