Job summary

Livingston Manor Inc. seeks a compassionate Registered Nurse (RN) to join our Home Health and Hospice team in Porter, TX, serving patients at home and in hospice settings. The RN will ensure high-quality care, patient safety, and comfort with strong clinical expertise and communication.

The role requires a TX RN license (BSN/ADN preferred), CPR certification, and reliable transportation. You will collaborate with physicians, families, and a multidisciplinary team, documenting in EMR and

Qualifications

• Current and valid RN license in Texas (or compact state).
• Graduate of an accredited School of Nursing (BSN or ADN).
• Current CPR (BLS) certification required.
• Valid Driver’s License, auto insurance, and reliable transportation required.

Responsibilities

• Conduct comprehensive initial and ongoing patient assessments in home settings.
• Develop individualized care plans with physicians, patients and families.
• Administer medications, IV therapies, wound care, and other skilled nursing procedures as ordered.
• Monitor, record, and report patient progress and changes to physicians and care team.
• Provide education to patients and families on disease management and home care techniques.
• Coordinate patient care with interdisciplinary teams.
• Ensure accurate and timely documentation in the EMR.
• Participate in on-call rotation as assigned.
• Maintain compliance with HIPAA and agency policies.
• Provide compassionate end-of-life care in hospice settings.
