Job summary

Livingston Manor Inc in Splendora, TX is seeking a compassionate Registered Nurse (RN) for Home Health & Hospice. You will provide skilled nursing care in patients' homes and hospice settings, ensuring safety, comfort, and high-quality outcomes.

Strong clinical skills, empathy, and clear communication are essential to collaborate with physicians, families, and the care team. The role is part-time/PRN with potential on-call duties; HIPAA compliance and accurate EMR documentation are required.

Qualifications

• Active RN license in Texas (or compact state).
• Graduate of an accredited nursing program (BSN/ADN).
• Current CPR (BLS) certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform initial and ongoing patient assessments in home/hospice settings.
• Develop individualized care plans with physicians and families.
• Administer medications, IVs, wound care, and other skilled procedures.
• Monitor and document patient progress; report changes to the care team.
• Educate patients and families on disease management and home care.
• Coordinate care with therapists, social workers, and aides.
• Maintain accurate EMR documentation; ensure compliance with HIPAA.
• Participate in on-call rotation as needed.
• Support end-of-life care with compassion and emotional support.
