Job summary

Visiting Nurse and Hospice for VT & NH in White River Junction, VT is seeking a Hospice Nurse Case Manager to deliver specialized home-based care. You will assess, plan, implement, coordinate, and evaluate patient care, developing targeted interventions and evaluating outcomes to meet each patient’s needs.

Candidates must hold an active VT and NH RN license and typically have an ASN or higher; BSN/MSN is preferred.

Qualifications

• Active RN license in VT and NH.
• ASN in Nursing required; BSN/MSN preferred.
• Weekend and on-call rotation required.
• Associates degree or higher in Nursing.
• Bachelor's degree in Nursing preferred.

Responsibilities

• Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.
• Supervise other home care staff as necessary.
