Job summary

Quality Home Health, Hospice & Palliative Care seeks an experienced Home Health and Hospice Admission Nurse to evaluate patients for admission, perform comprehensive assessments, and formulate initial care plans. You will coordinate services to ensure timely, appropriate home health or hospice care, acting as a liaison among patients, families, physicians, and the care team.

The role emphasizes strong assessment, education, and collaboration with interdisciplinary staff.

Qualifications

• Active RN license in good standing is required.
• Graduate of an accredited nursing program with a valid license.
• Experience in home health, hospice, case management, or admissions is preferred.
• Strong assessment, communication, and organizational skills are essential.
• Knowledge of Medicare, hospice, and home health regulations is required.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Perform initial patient assessments and admissions in home settings.
• Review records and orders to determine eligibility.
• Complete admission documentation accurately.
• Develop and implement individualized care plans.
• Educate patients and families on diagnoses, treatments, and services.
• Coordinate care with physicians, case managers, social workers, and therapists.
• Ensure compliance with Medicare, Medicaid, accreditation and agency rules.
• Evaluate needs and recommend services.
• Participate in on-call rotations as assigned.
• Maintain patient confidentiality and nursing standards.
