Job summary

Ascend Hospice seeks an Admissions Registered Nurse to perform initial patient and family assessments, determine home care needs, and coordinate admission processes. The RN will initiate plans of care, articulate hospice goals, and collaborate with the clinical team.

Ideal candidate has 2–4 years hospice experience, is licensed in the operating state, and may engage in hospital outreach to promote hospice services and coordinate patient transitions.

Qualifications

• Associate's Degree in Nursing required
• Bachelor's Degree preferred
• 2–4 years of hospice experience required
• Licensed RN in the state of operations

Responsibilities

• Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission
• Outline the patient/family referral and admission process
• Use assessment data to determine care plan needs and initiate the plan of care
• Articulate hospice philosophy, goals and objectives
• Coordinate care with other members of the clinical team
• Engage in hospital outreach to market hospice services

• Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission.
• Ability to outline the patient/family referral and admission.

• Articulates hospice philosophy, goals and objectives.
• Communicates with other members of the clinical team to coordinate the care plan.
