Pathways Healthcare, a leading home health and hospice organization, is seeking a Registered Nurse for our Home Health program in Concord, NH. The role is full-time with a Monday–Friday schedule from 8:30 AM to 5:00 PM, emphasizing patient-centered care and AI-assisted documentation to support clinicians.

The RN will manage care plans, perform assessments, coordinate with physicians, and ensure compliance with healthcare regulations while delivering compassionate, high-quality care in patients’

Qualifications

• Nursing degree from an accredited institution.
• 2+ years of nursing experience, preferably in geriatrics.
• New nurses welcome; training program available.
• Proficient with electronic medical records (EMR).
• Excellent bedside manner and communication skills.
• Knowledge of health care laws and regulations.

Responsabilités

• Evaluates and assesses patient status and initiates plan of care.
• Regularly re-evaluates patient and family needs.
• Develops and revises the care plan.
• Administers medications and treatments as prescribed.
• Maintains accurate clinical notes and tracks pain/symptom changes.
• Communicates with physician and follows orders.
• Ensures a safe patient environment and participates in group meetings.
• Participates in on-call duties and coordinates equipment/services.

Job Description:

• Role: Registered Nurse - Home Health
• Main Office: Concord, NH
• Schedule: Monday to Friday (8:30-5:00pm)
• Job Type: Full Time

Qualifications:

• Degree in Nursing from an accredited institution.
• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities:

• Evaluates and assesses patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
