Geisinger Home Health, part of LHC Group and aligned with Optum, seeks a Registered Nurse to provide and direct nursing care to patients in their homes, coordinating with physicians and the care team to maintain continuity of care.

The role emphasizes developing care plans, supervising aides and nurses, performing OASIS assessments, and ensuring compliant documentation, with opportunities for on-call rotations and a comprehensive benefits package.

Competenze

• Current and unrestricted RN licensure in state of practice
• Current Driver's License, vehicle insurance, and access to dependable transportation
• CPR certification preferred; Home Health experience preferred

Mansioni

• Initiates, develops, implements, and revises the plan of care with the physician and health care team
• Supervises care provided by home health aides and licensed nurses; provides instruction and assigns tasks
• Performs supervisory visits and ensures compliance with state/federal regulations
• Delivers high-quality clinical services within the plan of care and infection control standards
• Completes comprehensive assessments (OASIS) and medication reconciliation
• Documents visits per policy and payer requirements; coordinates with care team
• Communicates with physicians, patients, and families to ensure quality care
• Takes direction from Clinical/Executive Directors and participates in onboarding and survey readiness
• Participates in on-call and weekend rotation; supports utilization management
• Adheres to and supports the agency's quality improvement initiatives
