Qualifications

• Graduate of an approved school of professional nursing.
• Must maintain a valid clinical license as required by state.
• Minimum two years nursing experience within the last 36 months.
• One year of home health experience preferred.
• CPR certification required.
• Valid state driver license and automobile liability insurance required.
• Basic technology skills including mobile device use.
• Proficient in Microsoft Word and Excel.

Responsibilities

• Coordinate overall home health services to ensure care delivered per standards and procedures.
• Review and approve start of care, recertification, resumption of care, or evaluation data.
• Facilitate collaboration among physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and communicate with all stakeholders to maintain care continuity.
