Qualifications

• Current RN licensure in state of practice (Massachusetts).
• CPR certification required.
• Valid driver's license with access to a vehicle or reliable transportation.
• Ability to work with diverse patient populations and settings.

Responsibilities

• Provide clinical services within the scope of nursing practice per plan of care.
• Receive referrals, assign clinicians, start care visits.
• Coordinate determination of home health benefits and approvals.
• Assess patient needs and ensure care aligns with physician orders.
• Review assessments and care plans daily with clinicians.
