Job summary

Nurses at Home is seeking a dedicated RN Clinical Case Manager to deliver high-quality in-home health care, traveling to patient residences and prioritizing clinical excellence and compassionate service. You will work directly with families, coordinate treatments, and maintain precise documentation while upholding our culture of care.

The role is full-time with weekend on-call rotations, offering top industry compensation, health benefits, and two weeks of PTO.

Qualifications

• RN license required, valid in-state.
• Home health experience and OASIS proficiency preferred.
• Bachelor's degree preferred for nursing role.
• Willingness to travel and work on the road.

Responsibilities

• Provide in-home clinical case management and care coordination.
• Collaborate with families and care teams to develop individualized plans.
• Perform and document assessments and visits in patient records.
• Respond to patient and family needs with timely, compassionate care.
• Participate in weekend on-call rotations and after-hours coverage.
