Job summary

Enhabit Home Health & Hospice is seeking a qualified Registered Nurse to provide high-quality home health care. The RN will oversee patient care coordination across disciplines, perform OASIS assessments, and ensure timely communication with physicians and care teams.

A valid RN license and driving capability are required. Responsibilities include conducting visits, maintaining medication profiles, and participating in weekly and monthly care meetings to optimize patient outcomes in the home

Qualifications

• Active RN license in the state of practice.
• Minimum of one year of clinical nursing experience preferred.
• Valid driver’s license and automobile liability insurance are required.

Responsibilities

• Perform visits and coordinate client care in home health settings.
• Act as point of contact for disciplines involved in patient care.
• Consult with physicians and office staff to relay patient care details.
• Accurately perform OASIS data collection.
• Maintain up-to-date medication profiles and timely lab value reporting.
• Attend weekly case conferences and monthly case manager meetings.
