Unknown company in Newport Beach, CA, is seeking an RN Case Manager to lead patient care coordination and supervise a team of clinicians in a home health setting.

You will conduct comprehensive nursing assessments, develop and revise Plans of Care, and deliver direct skilled nursing care including wound care, medications, and IV therapy. OASIS assessments and EMR documentation are essential components of the role.

Qualifications

• Must be a licensed RN in the operating state.
• OASIS experience required; HCHB experience preferable.
• Graduate from an approved school of professional nursing and currently licensed to practice as an RN.
• One year experience as an RN.
• CPR certification required.

Responsibilities

• Conduct comprehensive nursing assessments during admission, resumption of care, recertification and discharge.
• Develop, implement, revise, and evaluate individualized Plans of Care with patients, families, physicians and IDT.
• Deliver direct skilled nursing care including wound care, meds, IV therapy and patient education.
• Perform accurate OASIS assessments to support reimbursement and outcomes.
• Educate patients and families on disease management and safety to prevent readmissions.
• Supervise LPNs/LVNs, aides, and support staff to ensure quality care.
• Monitor progress and adjust care plans as needed.
• Maintain EMR documentation per policies and regulatory requirements.
• Participate in case conferences and quality improvement initiatives.
• Manage caseload with efficient visits and ensure continuity of care.
• Respond to urgent needs and participate in on-call rotations as required.
• Serve as main contact for field clinicians and oversee utilization management review.
• Review clinical notes and supervise a team reporting to this role.

Overview

A home health agency is seeking an RN Case Manager to lead patient care coordination and oversee a team of clinicians in Newport Beach, CA.

What you'll do

• Conduct comprehensive nursing assessments of patients' physical, emotional, psychosocial, and spiritual needs during admission, resumption of care, recertification, and discharge
• Develop, implement, revise, and evaluate individualized Plans of Care in collaboration with patients, families, physicians, and the interdisciplinary team (IDT)
• Deliver direct skilled nursing care, including wound care, medication administration and management, IV therapy, injections, symptom management, and patient/caregiver education
• Perform accurate and timely OASIS assessments to ensure proper reimbursement, quality reporting, and optimal patient outcomes
• Educate patients and families/caregivers on disease management, medication adherence, safety measures, self-care techniques, and strategies to prevent re-hospitalizations and promote independence
• Supervise and direct LPNs/LVNs, home health aides, and other support staff to ensure coordinated, high-quality care delivery
• Monitor patient progress, promptly identify changes in condition, and adjust care plans accordingly
• Maintain accurate, timely clinical documentation in the Electronic Medical Record (EMR) system in compliance with agency policies, OASIS guidelines, and regulatory/payer requirements
• Actively participate in case conferences, interdisciplinary team meetings, and quality improvement initiatives
• Manage a productive caseload with efficient visit scheduling within the assigned territory while ensuring continuity of care and an exceptional patient experience
• Respond to urgent patient needs and participate in on-call rotations as required
• Serve as the main point of contact for field clinicians, including coaching and mentorship, responsibility for episode of care, order approvals, facilitating case conferences, and preparing/performing utilization management review
• Review clinical notes and oversee a team of clinicians reporting directly to this role

What you'll need

• Must be RN
• HH Leadership experience required
• OASIS experience required; HCHB experience preferable
• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
• One (1) year experience as a RN
• Licensed to practice as a registered nurse in the state of agency operation
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
• Ability to travel to all business locations
