Job summary

Elara Caring is seeking a Registered Nurse for Personal Care Services in Tulsa/Broken Arrow, Oklahoma. The role is full-time, Monday–Friday, with no on-call, weekends, or holidays, delivering high-quality home-based care.

You’ll lead care planning, supervise aides, and ensure compliance with regulations while supporting patients in their homes and traveling as needed. Join a compassionate team committed to excellent patient-centered care.

Qualifications

Graduate of an accredited School of Nursing.
Current RN license in good standing in the state applying for.
CPR Certification required.
Minimum of one year of nursing experience; Home Care experience preferred.
Knowledge of Medicare/Medicaid, home care benefits, and applicable State regulations preferred.
Strong clinical assessment, communication, and organizational skills.
Excellent computer proficiency and documentation abilities.
Ability to multitask and adapt in a fast-paced healthcare environment.
Professional verbal and written communication skills. Ability to motivate and lead caregivers.
Reliable personal transportation is required. Ability to travel locally, and regionally to support additional field offices as needed.

Responsibilities

Conduct start-of-care visits, supervisory visits, and client assessments to ensure quality care delivery.
Develop, review, and revise individualized plans of care based on patient needs.
Supervise homemakers, personal care aides, and home health aides to ensure compliance and quality standards are met.
Monitor and report significant changes in patients’ physical and mental conditions.
Educate caregivers, patients, and families using effective teaching and learning principles.
Ensure all services meet State, Federal, contract, and licensure requirements.
Conduct caregiver orientations and performance assessments as required.
Evaluate home environments for safety risks and recommend solutions to improve patient safety.
Maintain accurate clinical documentation, care plans, and service records.
Communicate professionally with caregivers, clients, case managers, and leadership teams.
