Job summary

Elara Caring is hiring a Registered Nurse for Home Health in Durant, OK. This part-time (PRN) role requires traveling to patient homes, performing assessments, and coordinating care with the clinical team.

The nurse will ensure adherence to the care plan and HIPAA privacy while delivering high-quality home-based care. Ideal candidates hold an unrestricted RN license, 1+ year of clinical nursing experience, and are willing to travel up to 50% within the service area.

Qualifications

• Current, unrestricted state RN license.
• 1+ year experience in a clinical care setting as a nurse.
• Ability to lift up to 50 pounds, assist with patient transfers.
• Willingness to travel 50% within branch coverage area.
• Associates or Bachelors degree in nursing preferred.
• Previous Home Health/HomeCare HomeBase exposure preferred.

Responsibilities

• Assess patient needs and develop, implement, and evaluate the plan of care.
• Communicate significant findings to the Clinical Supervisor and care team.
• Make initial evaluations and follow-up visits as required by patient needs.
• Travel to patient homes; maintain HIPAA privacy and confidentiality.
• Coordinate with physicians, staff, and families to optimize care.
