Job summary

Elara Caring is seeking a Registered Nurse (RN) Home Health in Tulsa, Oklahoma to deliver high-quality in-home care to patients across our coverage area. This ongoing Staff role emphasizes 1:1 patient care, mobility assistance, and flexible scheduling to fit your life.

You will hold an unrestricted Oklahoma RN license, travel up to 50% within branch coverage, lift up to 50 pounds, and collaborate with an interdisciplinary team.

Qualifications

• Current, unrestricted state RN license
• 1+ year experience in a clinical care setting as a nurse
• Able and willing to travel within branch coverage area
• Ability to frequently lift, push, pull, and support up to 50 pounds
• Must have dependable vehicle, valid driver's license and auto insurance
• Willing to travel 50%

Responsibilities

• Assesses assigned home health caseload to identify patient needs.
• Makes initial evaluation and follow-up visits per plan of care.
• Communicates findings to Clinical Supervisor, physician, and team.
• Revises the plan of care in response to issues.
