Job summary

Hackensack Meridian Health in New Jersey is seeking an experienced Registered Nurse to support telephonic case management and quality initiatives within our home health team. This role focuses on coordinating patient care and ensuring high-quality outcomes for patients receiving home health services.

You will collaborate with clinicians, document assessments in the EMR, and mentor nursing staff while ensuring adherence to clinical protocols and regulatory requirements.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of two (2) or more years RN Experience.
• Minimum of three (3) years certified home care experience.
• Excellent written and verbal communication skills.
• Proficient computer skills that include Google Suite and/or Microsoft Office platforms.

Responsibilities

• Assist clinical operation by providing telephonic case management and reports.
• Collaborate with specialists and facilities to provide home health services.
• Deliver nursing visits and manage caseload per nursing process.
• Prioritize patient visits with care coordinators and triage.
• Maintain medical records, conduct audits, and ensure OASIS accuracy.
• Mentor staff and participate in educational programs.
