Job summary

Hackensack Meridian Health at Home is seeking a nurse with home health experience to lead telephonic case management and coordinate patient care. You will work closely with physicians, support staff, and community resources to deliver skilled nursing services.

Responsibilities include conducting nursing visits, teaching patients and families, and ensuring accurate documentation and care plans in EMR systems. A valid NJ RN license and BLS are required.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing, required/preferred depending on role.
• Excellent written and verbal communication skills.
• Proficient computer skills incl. Google Suite and MS Office.

Responsibilities

• Provide telephonic case management and quality, patient satisfaction, and process-flow initiatives to the Manager of Clinical Practice.
• Coordinate care with specialists and facilities to deliver skilled home health services.
• Use nursing process to plan and deliver visits, educate patients and families, and ensure safe, cost-efficient care.
