Job summary

Hackensack Meridian Health at Home in New Jersey seeks an experienced Registered Nurse to support telephonic case management and home health services. You will collaborate with clinicians, coordinate care, and contribute to quality outcomes while following nursing processes and documenting care.

Requires Associate's Degree in Nursing or Diploma, 2+ years RN experience, 3+ years home care, valid NJ RN license, BLS, and comfort with EMR and Office tools.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing required.
• Minimum of two (2) or more years RN experience.
• Minimum of three (3) years certified home care experience.
• Excellent written and verbal communication skills.
• Proficient computer skills including Google Suite and Microsoft Office.

Responsibilities

• Responsible for assisting the clinical operation by providing telephonic case management as well as quality, patient satisfaction and process flow initiatives and reports to the Manager of Clinical Practice.
• Collaborates with specialists, other physicians, and professionals at local healthcare facilities and in the community, to provide home health services to patients in need of skilled nursing care.
• Utilizes the nursing process to provide nursing visits and/or manage a nursing caseload, and provide comprehensive care and teaching to patients and families.
• Assist client care coordinators in prioritizing patients in scheduling visits for case managers, through chart/document review and triage.
• Functions as the patient advocate in coordinating patient care; refers patients to community resources as needed.
• Attends patient care conferences and staff meetings; communicates patient needs to the team.
• Documents assessment data, care activities, interventions, and discharge plans within established timeframes.
• Identifies outcomes and reports significant or unexpected results of interventions to the physician.
• Identifies learning needs and initiates learning activities.
• Delegates tasks to ancillary personnel and ensures care is provided per care plan.
• Mentors staff and participates in educational programs and workstreams.
• Run and review productivity and clinical reports; conduct audits of patient records for QAPI.
• Maintain growth by attending educational programs and current literature review.
