Job summary

Hackensack Meridian Health at Home is seeking a registered nurse to support clinical operations through telephonic case management and quality improvement efforts. You will collaborate with physicians, specialists and community partners to deliver skilled nursing care in patients0homes, perform nursing visits, and educate families on individualized care plans.

The role requires an Associate's Degree in Nursing or a Diploma, 2+ years of RN experience, and 3+ years in home care.

Qualifications

• RN degree or nursing diploma required.
• Minimum 2+ years of RN experience.
• Minimum 3+ years certified home care experience.
• Excellent written and verbal communication skills.
• Proficient with Google Suite and/or Microsoft Office.

Responsibilities

• Provide telephonic case management and ensure quality, patient satisfaction and process flow.
• Collaborate with specialists and facilities to deliver home health services.
• Conduct nursing visits and manage caseload using nursing process.
• Educate patients and families on plan of care and medications.
• Document assessments, plans and discharge summaries consistently.
• Mentor staff and participate in audits and QAPI initiatives.
