Job summary

CHI Health at Home is seeking a Registered Nurse to provide patient-centered home health care, delivering high-quality, compassionate nursing in patients' own environments. You will assess, plan, and implement care that promotes independence and recovery.

Join a multidisciplinary team, mentor LPNs and aides, coordinate with physicians, and use technology to document care accurately. A valid RN license, CPR/BLS, and reliable transportation are required.

Qualifications

  • Graduate of an accredited nursing program with an active RN license in the state served.
  • Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
  • Exceptional critical thinking, proven clinical expertise, strong case management abilities.
  • Current CPR/BLS certification.
  • Valid driver’s license and reliable transportation.
