Job summary

Qualifications

• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care.
• Develop, implement, and adjust comprehensive care plans.
• Collaborate with physicians, therapists, and support staff for holistic care.
• Educate and empower patients and families with ongoing support.
• Mentor and guide LPNs, LVNs, and Home Health Aides on the caseload.
• Utilize technology to optimize care delivery and maintain accurate documentation.

As a Home Health RN, You Will

• Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.
• Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.
• Collaborate and coordinate. Work seamlessly with a multidisciplinary team - physicians, therapists, and support staff - to ensure holistic care.
• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

*Job Requirements*
