Job summary

CHI St. Vincent Health at Home is a Home Health RN role focused on delivering high-quality nursing care in patients' homes. You will develop individualized care plans, promote independence, and coordinate with a multidisciplinary team to ensure holistic care.

You will also educate patients and families, mentor LPNs, LVNs and Home Health Aides, and leverage technology to maintain precise documentation. This role supports professional growth and meaningful patient relationships.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering nursing care in the patient's home.

Job Requirements
