Job summary

Dignity Health at Home is seeking a Registered Nurse (RN) to deliver compassionate, high-quality care in patients’ homes. You will lead clinical activities, assess and adjust care plans, and collaborate with physicians, therapists, and support staff to achieve optimal outcomes.

The role requires an active RN license, at least 1 year of home health experience, CPR/BLS certification, and a valid driver’s license.

Qualifications

• Graduate of an accredited school of nursing.
• Must have an active RN license in the state served.
• Minimum of 1 year of Home Health nursing experience required.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge AI-powered clinical documentation and best practices to optimize care delivery and maintain precise documentation, promoting work-life balance and more patient interaction.

Job Requirements

No After Hours On-Call Requirements

Required

• Graduate of an accredited school of nursing.
• Must have an active RN license in the state served.
• Minimum of 1 year of Home Health nursing experience required
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Preferred

• Two years of hospital / hospice / homecare RN nursing experience preferred.

$37.40 - $55.63 /hour
