Job Description

The Registered Nurse will administer skilled nursing care for clients of all ages in their place of residence. This role includes coordinating care with the interdisciplinary team, the patient/family, and the referring agency while assuming responsibility for the coordination of care.

Responsibilities

• Assess home health and hospice patients in both stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) for quality and continuity of care.
• Gather data on physical, psychological, social, spiritual, and cultural factors influencing health status and incorporate it into the POC.
• Maintain up-to-date patient records including visits, contacts, problems, plans, actions, and goals.
• Provide care adhering to infection control measures and OSHA standards.
• Educate patients and families about care, diet, medications, and home safety, employing appropriate teaching methods.
• Supervise and guide HHA/STNA/LVN to ensure quality care.
• Assess and manage pain, providing education on pain management techniques to patients and families.
• Facilitate referrals to appropriate agencies as needed.
• Monitor assigned cases for compliance with third-party payer requirements.
• Perform on-call duties and provide support to patients and families as assigned.
• Communicate effectively with patients, families, team members, and other healthcare professionals.
• Participate in interdisciplinary team meetings and other necessary meetings.
• Perform other related duties as assigned.

Required Experience

• 1-2 years of experience as a Registered Nurse in a clinical care setting.
• 2+ years of home health experience.

Certifications/Licensure

• Current state license as a Registered Nurse - Required.
• CHPN certification - Preferred.
• Proof of current CPR certification - Required.
• Current driver’s license - Required.
• Reliable transportation and insurance - Required.
