Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year nursing experience; Home Health experience preferred.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient's home.
• Develop and adjust comprehensive care plans to promote independence and recovery.
• Collaborate with physicians, therapists, and support staff to ensure holistic care.
• Educate patients and families, providing crucial teaching and emotional support.
• Mentor and guide LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
• Utilize technology and best practices to optimize care delivery.

As a Home Health RN, You Will

• Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care.
• Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being.

Job Requirements
