Job summary

CommonSpirit Health at Home in Durango, CO seeks a Hospice RN to provide home-based nursing care with a 1:1 patient-clinician ratio. You will craft individualized care plans, collaborate with a multidisciplinary team, and educate families to support healing and independence.

You will mentor Home Health Aides, leverage technology for care delivery, and ensure comprehensive documentation. RN license, 1 year of nursing experience, CPR/BLS, and driving ability required.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience; Home Health experience is a plus.
• Strong critical thinking and proven clinical expertise with case management abilities.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient’s home.
• Develop and adjust comprehensive care plans to promote independence and well-being.
• Collaborate with physicians, therapists and support staff for holistic care.
• Educate patients and families, delivering crucial teaching and emotional support.
• Mentor LPNs, LVNs, and Home Health Aides to foster a cohesive team.
• Utilize technology to optimize care delivery and maintain precise documentation.

As a Hospice RN, You Will

*Job Requirements*

• Graduate of an accredited nursing program with an active RN license in the state served.
