Job summary

CommonSpirit Health at Home is seeking an experienced Home Health RN to deliver high-quality nursing care in patients' homes across Colorado Springs. You will lead clinical care in a patient-friendly setting, developing and adjusting comprehensive care plans while collaborating with physicians, therapists, and support staff.

This role emphasizes patient-centered care, mentorship of LPNs/LVNs, and thoughtful of technology to improve outcomes.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience. Home Health experience required.
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
