Job summary

CommonSpirit Health at Home seeks a compassionate Hospice RN to lead in-home nursing care, develop tailored care plans, and coordinate with a multidisciplinary team. You will educate patients and families, mentor junior staff, and leverage technology to enhance care and documentation.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient’s home environment.
• Craft individualized care plans and dynamically adjust them to promote independence and well-being.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Educate and empower patients and families with crucial teaching and support.
• Mentor and guide caseload, including LPNs, LVNs, and Home Health Aides to build a cohesive team.
• Innovate with technology and best practices to optimize care delivery and documentation.

As a Hospice RN, You Will
