Job summary

CommonSpirit Health at Home is seeking a compassionate RN to deliver high-quality, in-home nursing care in Westminster, CO. You’ll manage a 1:1 patient-clinician relationship, develop tailored care plans, and collaborate with a multidisciplinary team to support patient recovery.

Qualifications

• Graduate of an accredited nursing program with an active RN license.
• Minimum of 1 year of Home Health nursing experience.
• Strong critical thinking and clinical expertise.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide high-quality, personalized nursing care in patients' homes.
• Develop and adjust comprehensive care plans supporting independence.
• Collaborate with physicians, therapists, and support staff.
• Educate patients and families, fostering understanding and empowerment.
• Mentor and guide LPNs, LVNs, and Home Health Aides on caseloads.
• Leverage technology to optimize care delivery and documentation.

As a Home Health RN, you will:

Job Requirements

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
