Job summary

Inspiritus Inc. is seeking a Home Care Nurse (RN) to perform monthly in-person assessments for individuals with I/DD in Atlanta/South. You will collaborate with the clinical service team and report to the Nursing Services Coordinator.

This PRN role requires travel to clients’ homes and day programs, with a flexible schedule and duties including documentation and education. The ideal candidate holds a current Georgia RN license, a valid GA driver’s license, and has 2 years of nursing experience,

Qualifications

• RN license or higher clinical license is required.
• GA driver's license in good standing; current auto insurance.
• 2 years’ experience; 3–5 years working with individuals with I/DD preferred.
• BLS/First Aid certification is required or must be obtained.
• CPI certification is preferred.
• Knowledge of DBHDD rules and person-centered planning is preferred.

Responsibilities

• Conduct comprehensive in-person assessments of clients’ physical, mental, and psychosocial needs.
• Provide person-centered education to individuals and Host Home Providers; train as required.
• Document in Electronic Health Record system as directed.
• Travel to client homes and day programs for assessments.
