Job summary

The Home Care Nurse (RN) at Inspiritus supports clients eligible for nursing services. The role conducts monthly in-person assessments for individuals with I/DD, collaborates with the clinical team, and travels to client homes or day programs.

A flexible PRN schedule applies, with oversight by the Nursing Services Coordinator. Responsibilities include assessments, documentation, education, and clear communication; travel to service sites is required across Atlanta/Northeast/East regions.

Qualifications

• Valid and current RN license.
• Valid Georgia driver's license with vehicle insurance.
• 2 years’ experience; 3-5 years with individuals with I/DD preferred.
• Comprehensive nursing assessment experience and care planning.
• Knowledge of DBHDD rules and person-centered planning preferred.
• BLS/First Aid Certification and CPI Certification preferred.

Responsibilities

• Conduct comprehensive in-person assessments of clients' needs.
• Provide person-centered education to clients and Host Home Providers.
• Document assessments in the EHR system as directed.
• Travel to clients' homes and day programs for assessments.
• Report findings to the Nursing Services Coordinator and team.
