Position Overview

The primary function of the Clinic/Homecare RN is to assess, coordinate, monitor, and provide healthcare services and treatment for Bluestem PACE participants. This includes skilled nursing, palliative care, planning and monitoring in-home services, and supervising home care aides, along with supporting quality improvement, data collection, and analysis.

Essential Job Functions
  • Provide direct patient care in home and clinic settings according to policies and regulations.
  • Conduct assessments and participate in interdisciplinary team meetings.
  • Manage nursing and palliative care needs of assigned participants.
  • Facilitate integration of new participants into the care system, including medication and routine monitoring.
  • Perform ongoing assessments and report data as required.
  • Coordinate with primary care providers and assist in service coordination.
  • Provide health education and counseling to participants and caregivers.
  • Report condition changes promptly and follow up as needed.
  • Administer medications and treatments as ordered.
  • Develop and update care plans based on nursing assessments.
  • Ensure documentation is complete, accurate, and timely.
  • Participate in quality assurance and safety initiatives.
  • Assist with adherence to safety protocols and infection control.
  • Attend staff meetings and participate in organizational activities.
  • Coordinate admissions, discharges, and transfers with other departments.
  • Provide off-hour on-call assistance as assigned.
  • Use personal vehicle for work-related travel.
Qualifications and Experience
  • Must be at least 18 years old.
  • Registered Nurse license from an accredited program.
  • Proficiency in Microsoft Office Suite.
Physical Requirements

Medium Work – exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds frequently, and/or greater than negligible up to 10 pounds constantly.
